Taxi driver medical

You will need the local authority’s (where you intend to work) version of the Taxi Driver Medical form which is specific to them – it is similar to the D4 Form.  We will then complete the medical and the form for you, stamp and sign it and keep a copy for our records.

Photo ID will be needed to support your application along with a copy of your full medical history which you will need to request from your GP practice.
